The form issued by the Dubai Health Authority (DHA) has to be filled and submitted if a student or his or her close relative has visited China, Hong Kong, South Korea, Iran, Japan, Singapore or Italy in the last 28 days.
According to authorities, this will enable them to help alert relevant medical teams if a child approached the school clinic with flu-like symptoms.
Earlier, the UAE Ministry of Education advised students returning from China to stay at home for at least 14 days
